Student Athlete Forms

All forms must be completed in full each year and received by the Athletic's staff prior to your first practice with an intercollegiate team. The form can be mailed and must be received no later than your designated due date if your first practice takes place before the start of classes for the fall semester. For all other athletes, please refere to the due dates below. 

Due Dates

• Football - August 1st

• Field Hockey, Cross Country, and Soccer - August 8th

• Fall Baseball, Lacrosse, and Softball - August 15th

• Ice Hockey, Basketball, and Track - September 15th

• Forms submitted after the due dates will be reviewed as they are received, but medical clearance cannot be guaranteed by the first practice.
